On or about April 25, 2022, at the above addressed jobsite, employees were exposed to struck-by hazards while lifting masonry material by the direct placement of rigging equipment (i.e., sling) onto the tines of a SKYJACK SJ1256 THS Telehandler for a below-the-tines lift, which could affect the capacity and/or safe operation of the truck. The employer did not receive written approval from the manufacturer of the truck before using the sling.
