29 CFR 1910.147(f)(3)(ii)(D):Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: Walnut Hill Feeds, Inc. located at 6048 State Route 528 in Shelby, Ohio 44875; On or about April 25, 2022, the employer did not ensure that procedures were utilized while engaged in die changes and plug clearing of pellet mills which affords each employee involved a level of protection equivalent to that provided by the implementation of a personal lockout device. Employees were exposed to caught in and amputation hazards in that each employee engaged in a die change on the Andritz Pellet Mill (Model 18X56DD) did not apply a personal lock out tag out device.

29 CFR 1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: Walnut Hill Feeds, Inc. located at 6048 State Route 528 in Shelby, Ohio 44875; On or about April 22, 2022, On or about May 18, 2022, the employer did not ensure that employees were provided with information contained within Appendix D of CFR 1910.134 Respiratory Protection Standard. Employees handling feed ingredients were provided with tight fitting N95 filtering facepiece respirators to be worn voluntarily. Employees were exposed to hazards associated with various feed additives such as but not limited to, copper sulfate and powered minerals.
