29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident: a) Walmart, Inc., 7500 E. Crossroads Blvd, Loveland, CO 80538: The employer did not ensure that employees were retrained, and evaluated on said training, after experiencing near-miss incidents or being involved in an accident. This exposes employees to struck-by and caught-between hazards. WALMART, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard , which was contained in OSHA inspection number 1547108, citation number 1 , item number 1 and was affirmed as a final order on 12/13/2021, with respect to a workplace located at 6785 SW Enterprise Blvd, Walmart Distribution Center 7023, ARCADIA, FL 34269.
