Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a. On or about April 26, 2022, at the above addressed jobsite, employees conducting roofing operations, 20'-7" and 19'-3" above the ground, were not protected from falls.

On or about April 26, 2022, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
