29 CFR 1910.28(b)(3)(ii): The employer did not ensure that each employee be protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems: a. Colorado Stone Quarries, Inc - Delta Logistics Yard at 1734 Hwy 50 E., Delta, CO 81416, the employer did not ensure employees were protected from injury in that the walking area in the Quality Assurance Line was not free from slip and trip hazards. The water trough in the ground was not covered and exposed employees to slip and trip hazard.
