29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite: On or about April 27, 2022, employees were using two different ladders (10 foot stepladder and a 13 foot section of an extension ladder) to reach different levels of the residential construction site. Neither ladder extended at least 3 feet about the ladder landing.

29 CFR 1926.1060(b):The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: (a) Jobsite: On or about April 27, 2022, the employer needs to provide retraining in ladder usage to employees. Two employees were using ladders that were set up improperly.
