29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about April 26, 2022 at 205 Kanis Ridge, employees performing roofing activities used ladders to gain access onto/off the roof and the ladders were not extended at least 3 feet above the upper landing surface. These conditions exposed the employees to fall from height hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about April 26, 2022, employees installing shingles at 205 Kanis Ridge, Little Rock were operating 1-3/4 Inch Coil Roofing Nailer, Metabo HPT NV45AB2 with no eye and/or face protection. The Javier Aquino was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number __1287567__, citation number__1__, item number__1__ and was affirmed as a final order on February 6, 2018, with respect to a workplace located at__14810 Pride Valley Rd, Little Rock, AR 72223__.
