On or about April 25, 2022 on the east side of the feed mill, employee(s )operating the bulk feed trailer 6113 were exposed to struck by, caught in, and fall hazards in that the employer did not remove the truck from service when it was believed the air valve to the safety rail actuator was defective on the upper walking-working surface.

29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. On or about April 25, 2022 on the east side of the feed mill, employee(s )operating the bulk feed trailer 6113 were exposed to struck by, caught in, and fall hazards in that the employer did not remove the truck from service when it was believed the air valve to the safety rail actuator was defective on the upper walking-working surface.
