29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration. Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  (a) Location: Jobsite at 11 Central Street, Salem, MA.  On or about 4/27/2022, workers were exposed to fall hazards while replacing exterior corbels, at the roof fascia, from a scaffold that was not erected under the supervision of a competent person.

(a) (a) Location: Jobsite at 11 Central Street, Salem, MA.  On or about 4/27/2022,  employees were exposed to fall hazard while working on pump jack scaffolding greater than 20 feet above the lower level.

29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  (a) Location: Jobsite at 11 Central Street, Salem, MA.  On or about 4/27/2022, the employer allowed employees to work from a pump jack scaffold with 2x14 in planks containing knots and other imperfections. These scaffold planks were not graded by a recognized lumber grading association for this use and were not capable of supporting 4 times the maximum intended load.
