29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee was using appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a)  On April 26, 2022  employees were using pneumatic nail guns to install roofing shingles, and were not equipped with and required to wear eye protection.

29 CFR 1926.501(b) (13):  The employer did not ensure that each employee engaged in residential roofing activities with unprotected sides and edges (6) feet or more above lower levels, was protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.   a) On April 26, 2022, employees engaged in residential roofing activities were exposed to falls of approximately 11 feet.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  a) On or about April 26, 2022, the employer did not maintain records or certification of fall protection training for employees exposed to fall hazards.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
