29 CFR 1910.134(f)(2):The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter. (a) On or about April 28, 2022, all employees who were required to wear N 95 tight fitting respirators (Honeywell DC 300, Honeywell TC-84A-8139, 3M 1860S, 3M 8210) in the workplace during COVID-19 outbreaks were not fit tested prior to initial use of the respirator.

When an authorized government representative asks for the records you keep under Part 1904, you must provide copies of the records within four (4) business hours. (a) On April 28, 2022, the employer did not provide OSHA 300 Logs and 300 A Summaries for the calendar years 2018, 2019, 2020, 2021 and 2022 year to date, within four hours of the request at 10:00 AM.
