On or about 04/28/2022 at 134-03 35th Avenue, 13th floor, Flushing, NY 11354. A floor hole cover measuring approximately 16'' x 3', that was made of lumber, was not secured. B.) On or about 04/28/2022 at 134-03 35th Avenue, 14th floor, Flushing, NY 11354. A floor hole cover measuring approximately 16" x 3', that was made of lumber, was not secured. ABATEMENT CERTIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
