29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  On or about April 27, 2022, and at times prior, the employer exposed employees to flying chips and debris in that  Bosch brake lathe #BL8922L, located in the automotive shop area, was not guarded at the point of operation.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  (a)  On or about April 27, 2022,  and times prior, the employer exposed employees to stuck-by hazards due to the DeWalt DW756, bench grinder work rest, located in the shop area, not being adjusted to one eighth inch.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  (a)  On or about April 27, 2022, and times prior, the employer had not developed and/or implemented a written hazard communication program to address the chemicals used in the workplace and the hazards associated with those chemicals.   A written hazard communication program shall include the following elements, as applicable:  1. An inventory of all hazardous chemicals used in the workplace; 2. Labeling of chemicals; 3. Maintaining of Safety Data Sheets for all chemicals, ensuring accessibility for employees; 4. Training; 5. Periodic review to address changes in the workplace.  Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a) On or about April 27, 2022, and at times prior, the employer did not ensure employees were protected from chemical hazards where two containers of engine coolant were not labeled, tagged or marked with product identifiers.
