This exposed employees to a caught-on and caught-in hazard. (b) On or about April 28, 2022 at the 45 coffee bean roaster, a vertical drive belt and pulley assembly with exposed spokes located approximately six (6) inches above the floor was not properly guarded. This exposed employees to a struck-by and caught-on hazard. (C) On or about April 28, 2022, at the 90 coffee bean roaster, a horizontal and vertical drive belt, and pulley assembly located approximately Five (5) feet above the floor was not properly guarded. 29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: (a) On or about April 28, 2022, at the 230 volt 3 phase ventilation fan control panel and disconnect located in the rear of the coffee bean roasting room, an unused opening in the front of the electrical panel was not plugged or otherwise covered. This exposed employees working at or around the panel to an electrical shock hazard. (b) On or about April 28, 2022, at the 110 volt receptacle outlet used to supply electrical energy to the portable tankless water heater located below the employee hand wash sink, did not have a cover plate to prevent accidental access to the electrical wires and connections to the receptacle outlet. This exposed employees working at or around the receptacle to an electrical shock hazard. (c) On or about April 28, 2022, at the 110 volt receptacle outlet used to supply electrical energy next to the coffee grinder in the back of the coffee bean roasting room, did not have a cover plate to prevent accidental access to the electrical wires and connections to the receptacle outlet. This exposed employees working at or around the receptacle outlet to an electrical shock hazard.

29 CFR 1910.303(g)(1)(i)(B): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the width of the working space in front of electrical equipment was less than the width of the equipment or 762 mm (30 inches), whichever was greater : (a) On or about April 28, 2022, adequate access area was not provided to allow employees emergency access to the electrical panels located at the back of the roasting room. Fifty (50) gallon plastic cans used for bean storage were kept in front of the control panels. This condition did not allow sufficient entrance and access to the two 230 volt electrical panels/disconnects. (b) On or about April 28, 2022, adequate access area was not provided to allow employees emergency access to the electrical circuit breaker panels located in the storage room. Pallets were stored in front of the electrical breaker box, not allowing sufficient entrance and access to the electrical panel.
