At 185 Matanzas Avenue, Saint Augustine, Florida;  On or about April 26, 2022, an employee was working on the ground below other employees installing green board and was not wearing a hard hat, the employee was exposed to head injury from falling tools and or debris from the workers above.

At 185 Matanzas Avenue in Saint Augustine, Florida 32084;  On or about April 26, 2022, the employer exposed employees to a fall hazard, in that employees were working out of an aerial lift without fall protection.

At 1311 NW 13th Street, Gainesville, Florida 32601;  On or about April 27, 2022, the employer exposed employees to fall hazards, in that employees performing finishing work from an aerial lift had not been trained to recognize fall hazards.
