29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.  At 185 Matanzas Avenue, Saint Augustine, Florida;  On or about April 26, 2022, an employee was working on the ground below other employees installing green board and was not wearing a hard hat, the employee was exposed to head injury from falling tools and or debris from the workers above.

29 CFR  1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.  a. At 185 Matanzas Avenue in Saint Augustine, Florida 32084;  On or about April 26, 2022, the employer exposed employees to a fall hazard, in that employees were working out of an aerial lift without fall protection.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.   a. At 1311 NW 13th Street, Gainesville, Florida 32601;  On or about April 27, 2022, the employer exposed employees to fall hazards, in that employees performing finishing work from an aerial lift had not been trained to recognize fall hazards.
