On or about 4/29/2022, at the jobsite located at 1703 Palm Beach Lakes Blvd, West Palm Beach, FL 33401, an employee was exposed to an amputation hazard while working with a food slicer (Model #G12, Serial #52_865) without wearing a chain glove on the right hand.
