On or about April 29, 2022 and at times thereafter, employees used powered air purifying respirators and the employer did not did ensure employees inspected the filters before each use and during cleaning.

29 CFR 1910.134(k)(5)(ii): Retraining was not conducted when inadequacies in the employee's knowledge or use of the respirator indicated that the employee did not retain the necessary training or skill: UnityPoint Health Methodist located at 221 NE Glen Oak in Peoria, IL. On or about April 29, 2022 and at times thereafter, employees used powered air purifying respirators and the employer did not ensure employees were retrained to inspect the powered air purifying respirator filters to ensure it is present, the filter and gasket are seated properly and are not damaged and has not become wet when employees had not retained the training.
