29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about April 29, 2022, located at the jobsite, where employees performing roofing work were using a warning line system without a combination of different fall protection systems.

29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which is parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which is perpendicular to the direction of mechanical equipment operation. a) On or about April 29, 2022, located at the jobsite, where employees performing roofing work were using a warning line system near the hoisting equipment was not set-up at the proper distance.
