29 CFR 1926.502(d)(17):The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Location - 72 MacIntosh Ln. Boxborough, Ma On or about 4/29/2022, the employer did not ensure employees utilizing PFAS were correctly attaching their lifelines. The employees were attaching at the hip/front dee-ring (mountain climbing) instead of the dee-ring located in the middle of the shoulder area.

Boxborough, Ma On or about 4/29/2022, the employer did not ensure employees utilizing an extension ladder were protected from possible falls to a lower level by installing a fiberglass ladder correctly.
