29 CFR 1910.178(m)(1): Trucks shall not be driven up to anyone standing in front of a bench or other fixed object. On or about April 27, 2022, an employee operating a rough terrain forklift drove the machine's forks directly in front of an employee's mid-section who was standing between a drill bit and a trailer. This exposed an employee to a struck-by hazard.

29 CFR 1910.304(c)(2): Clearance from ground. Open conductors, open multiconductor cables, and service-drop conductors of not over 600 volts, nominal, shall conform to the minimum clearances specified in Table S-6. Open conductors, open multiconductor cables, and service-drop conductors of not over 600 volts, nominal, shall conform to the minimum clearances specified in Table S-6. On or about April 27, 2022, Cactus Rig #171, black electrical cables (120-125V) were not at a minimum height clearance of 10 Feet. This condition exposed employees to struck-by hazards and electrical shock hazards in the following instances: a) Black power cord (120-125V) running from diesel storage tank to engine room was approximately 5'8" from the ground. b) Black power cord (120-125V) running from diesel storage tank to storage container had approximately 4'7" from the ground.

This condition exposed employees to chemical burn hazards in the following instances: a) Battery located in the VFD Variable Frequency Drive house. b) Battery used for cold start for air compressors.
