29 CFR 1910.305(b)(3)(ii): Pull and junction boxes used on systems rated over 600 volts, nominal, were not closed by suitable covers securely fastened in place: On or about April 27, 2022, Cactus Rig #171; Trailer #67277 a 240-voltage electrical box cover had an opening drilled into the cover . This exposed employees to electrical shock hazards.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): On or about, April 27, 2022, a damaged extension cord was being utilized from the water pump to the crew trailer. This condition exposed employees at the rig site to electrical hazards such as electrical shock and/or burns.
