29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: a) Jobsite, 2984 Sidney Street, Pittsburgh, PA: On or about May 2, 2022, an employee was working out of an aerial lift approximately 20 feet in the air and did not have the lanyard attached to the lift. b) Jobsite, 2984 Sidney Street, Pittsburgh, PA: On or about May 2, 2022, an employee was inside an aerial lift, coming down to the ground from approximately 20 feet above the ground and was not wearing or utilizing any type of fall protection.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) Jobsite, 2984 Sidney Street, Pittsburgh, PA: On or about May 2, 2022, an employee was painting on the 3rd floor balcony while standing on the 3rd step of a 6 foot portable ladder that exceeded the height of the guardrail present on the balcony and was not utilizing any other type of fall protection. The fall hazard was approximately 30 feet to the ground below.

The owner said he didn't have the written fall protection training documents for him.
