29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  a) Rose Roofing & Restoration, on or about May 02, 2022, at the address of 85 Mendota St. Mansfield, OH: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns during roofing work on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) Rose Roofing & Restoration. On or about May 02, 2022, at the address of 85 Mendota St. Mansfield, OH: The employer did not ensure employees were protected from falls while working on top of a two-story residential 8 /12 pitch roof. Employees laying new shingles from atop the structure in this condition were exposed to falls up to 19 ft. to the ground below.

29 CFR  1926.1053(b)(1):Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a) Rose Roofing & Restoration. On or about May 02, 2022, at the address of 85 Mendota St. Mansfield, OH: The employer did not ensure employees using fiber glass ladders for access had their side rails extended at least three feet above the landing surface. Three employees were exposed to fall hazards while using ladders to access a two-story residential structure with a roof eave height of  19ft  6 inches from ground below and an 8 /12 roof pitch.
