29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Capital C Masonry located at 3032 Avalon Road, Upper Arlington, OH 43221: On and preceding May 2, 2022, the employer did not ensure that employees working in an area where there was possible danger of head injury from falling or flying objects were protected by protective helmets. A masonry crew was working on a two tier scaffold with tools staged on the scaffolding approximately 14 feet above ground level tossing materials to the ground. Employees were walking/working in the area directly beneath the scaffold without head protection thereby exposing employees to struck-by hazards.

A scaffolding platform that was in use for masonry work on a multi-story residential home had sections throughout the second tier of the platform that did not have guardrails installed thereby exposing employees working on scaffolding to falls up to 14 feet.
