On or about April 18, 2022, and at times prior thereto, employees were working on a flat roof without the use of a guardrail system, personal fall arrest system, or other acceptable form of fall protection, exposing the employees to the hazard of falling up to approximately 30-feet to the concrete floor below.

29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On April 18, 2022, at approximately 2:41pm, an employee suffered an injury that resulted in hospitalization. The hospitalization was not reported within 24-hours to the local Area Office of the Occupational Safety and Health Administration. The incident was first reported to the OSHA Houston North Office on April 25, 2022 and then reported to the OSHA Austin Office on April 26, 2022.
