29 CFR 1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At a worksite located at 809 Savannah Dr, Greenwood, MO 64034, an employee was exposed to electrical hazards that included electric shock and electrocution. The employer exposed the employee to these electrical hazards by allowing him to use an electrical extension cord that had damage related to improper strain relief.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). At a worksite located at 809 Savannah Dr., Greenwood MO, 64034, employees were exposed to a fall hazard greater than six feet. The employer exposed the employees to this fall hazard by allowing them to work at heights greater than six feet without the use of fall protection or mitigation measures.
