29 CFR  1910.178(q)(6): Powered industrial trucks were altered so that the relative positions of the various parts were different from what they were when originally received from the manufacturer:   a)  Warehouse:  On or about May 03, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that the employer inverted the fork attachment used in combination with a crane attachment on the "Hyster" forklift, model S120XL2S (serial number D004V09074S) where employees use the forklift to transport stone slabs.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  a) Warehouse:  On or about May 03, 2022, and at times prior, the employer exposed employees to struck-by hazards, in that the employer provided practical powered industrial truck operator training but not formal powered industrial truck operator instruction to employees who operate the "Hyster"  model S120XL2S forklift  (serial number D004V09074S) to transport stone slabs.
