On or about May 03, 2022, at a residential construction site located on 5023 Sunway Dr. in Katy, TX, an employee was exposed to eye injuries while operating a pneumatic nail gun without wearing eye protective equipment.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about May 03, 2022, at a residential construction site located on 5023 Sunway Dr. in Katy, TX, employees were exposed to a fall hazard when using an extension ladder to access the roof without the side rails at least three feet above the roof landing.
