Gibson Area Hospital and Health Services located at 1120 N Melvin Street in  Gibson City, IL.   On or about February 3, 2022 and times thereafter, the employer did not provide a medical evaluation to determine each employee's ability to use a respirator before requiring the use. The employer required employees to wear N95 respirators while near suspected and confirmed positive COVID-19 patients.

29 CFR  1910.134(f)(2): The employer shall ensure that an employee using a tight-fitting facepiece respirator is fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) is used, and at least annually thereafter.   Gibson Area Hospital and Health Services located at 1120 N Melvin Street in  Gibson City, IL.   On or about February 3, 2022 and times thereafter, the employer did not ensure employees using tight-fitting facepiece respirators were fit tested prior to initial use of the respirator and at least annually thereafter.  The employer required employees to wear N95 respirators while near suspected and confirmed positive COVID-19 patients.

On or about February 3, 2022 and times thereafter, the employer did not prohibit employees with facial hair that interfered with the seal from wear a tight fitting respirator.
