29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or before May 1, 2022, and at times prior, the employer did not ensure that each employee(s) engaged in residential construction activities 6 feet or more above lower levels were protected by guardrails, safety nets, or personal fall arrest systems. An employee of LA Construction LLC was exposed to fall hazards greater than 10 feet, while engaged in residential roofing activities. Employees were not protected by a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or before April 30, 2022, and at times prior, an employee accessed the roof of a residential house using an extension ladder. The side rails of the ladder was not extended at least 3 feet above the landing surface of the roof. This condition exposed the employee to fall hazards.
