Maumee OH 43537, did not provide a training program for each employee potentially exposed to fall hazards while using a portable ladder to enable each employee to recognize the hazards of falling from the ladder and the procedures to be followed in order to minimize these hazards. Four employees using the single aluminum extension ladder to access the roof to perform roofing work were exposed to fall hazards 9 feet or greater to the ground below.
