29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Worksite: The employer did not ensure that the extension ladder that employees were required to use to gain access to the roof at a height of 16 feet above the ground had its side rails extended 3 feet above the landing surface.

29 CFR  1926.1060(a)(1):1926.1060(a)(1):  The employer did not ensure that each employee was trained by a competent person which would enable each employee to recognize hazards related to ladders and stairs, and the procedures to follow to minimize those hazards.  Worksite: The employees who were required to use the extension ladder to gain access to the roof at a height of 16 feet above the ground were not trained by a competent person in the proper procedures to minimize the hazards associated with ladders.

Worksite: The employer did not ensure that the employees who were working on the residential roof were protected from fall hazards above six feet by the use of guardrails, safety nets or personal fall arrest systems.  Perez Diaz Contracting, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1301862, citation number 1, item number 1a and was affirmed as a final order on 4/16/2018, with respect to a workplace located at 171 Edgewood Road, Fairfield, CT.
