On May 4, 2022, three NorthStar employees were injured when a concrete mezzanine platform unexpectedly collapsed during demolition and abatement operations. One employee suffered a double leg imputation. On or about May 4, 2022, and times prior; Suffolk Construction, failed to provide frequent and regulars inspections inside the asbestos control area of building 1A west wall mezzanine.

Location: Worksite On or about May 4, 2022 and times prior, employees were exposed to a floor collapse hazard while working where demolition and asbestos debris was being stored on a mezzanine floor where the safe load limit was not conspicuously posted.

29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey was not made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure, including adjacent structures where employees may be exposed. The employer did not have in writing evidence that such a survey was performed. Location: Worksite On May 4, 2022, three NorthStar employees were injured when a concrete mezzanine platform unexpectedly collapsed during demolition and abatement operations. One employee suffered a double leg imputation. On or about May 4 2022, Suffolk Construction did not have a plan in place to prevent an unplanned collapse of the Building 1A west wall mezzanine.
