Oleksa, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1556680, citation number 2, item number 1 and was affirmed as a final order on November 30, 2021, with respect to a workplace located at 2212 Emerson Street, Evanston, IL 60201.  Oleksa, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1245854, citation number 1, item number 3a and was affirmed as a final order on March 5, 2018, with respect to a workplace located at 12926 Sunrise Drive, Lemont, IL 60439.
