29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  a. Located at 13894 Sunladen Court NW in Pickerington, Ohio: On or about May 4, 2022, employees were using a multi-position ladder placed against the first story roof eave to access the roof. The top of the ladder did not extend at least three feet above the roof, thereby exposing employees to a fall hazard.

29 CFR  1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  a. Located at 13894 Sunladen Court NW in Pickerington, Ohio: On or about May 4, 2022, employees were performing re-roof work on the roof of a two-story residential home at heights in excess of twenty feet exposed to falls and struck-by hazards. The employer did not maintain an effective safety and health program related to the roofing work to ensure that the hazards were identified and corrected onsite.   Campa Roofing, LLC was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1), which was contained in OSHA inspection number 1427271, citation number 1, item number 3b and was affirmed as a final order on 10/02/2019, with respect to a workplace located at 713 Parsons Avenue, Columbus, Ohio 43206.
