29 CFR  1926.20(b)(2): The employer did not ensure that programs provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 1201 Flinn Highway, Glenshaw, PA: On or about May 6, 2022, the employer did not ensure that a program of frequent and regular safety and health inspections was implemented to identify and correct unsafe working conditions. Deficiencies noted during the inspection included, but were not limited to, failing to use eye/face protection while using powered equipment such as circular saws or nail guns, not following proper ladder safety practices, failing to use fall protection in areas such as the top of the building, permitting the use of a bobcat skid steer forks/load on the forks as a work platform and not ensuring that electrical cords were free from recognized hazards.

29 CFR  1926.1053(b)(1):  When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access; or, when such an extension was not possible because of the ladder's length, then the ladder was not secured at its top to a rigid support that would not deflect, and a grasping device, such as a grabrail, was not provided to assist employees in mounting and dismounting the ladder:    a) Job site, 1201 Flinn Highway, Glenshaw, PA: On or about May 6, 2022, the employer did not ensure that a ladder used to gain access to/egress from the top of the building had it's side rails extended at least 3 feet above the landing surface, exposing employees to fall hazards of up to approximately 16 feet.

Deficiencies noted during the inspection included, but were not limited to, failing to ensure that ladders used for access/egress purposes extended at least 3 feet above the landing surface or that they were tied off and a grab bar provided, allowing employees to cross over horizontally to/from the ladder from an adjacent surface in the air and not ensuring that ladders were properly maintained/not damaged.
