29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   a. On or before May 4, 2022, the employer failed to establish, implement and utilize an energy control procedure for the Line 1 auger. An employee was cleaning out residual material from the auger without utilizing a documented procedure and without applying a lockout device to isolate the hazardous energy source. During this covered activity, the employee was exposed to caught-in hazards from the danger zone of the auger.

29 CFR  1910.147(c)(7)(i)(A):Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  a. On or before May 4, 2022, the employer failed to train employees on the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. An employee was cleaning out residual material from the auger without having been trained on energy isolation and control. During this covered activity, the employee was exposed to caught-in hazards from the danger zone of the auger.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or before May 4, 2022, the employer failed to ensure employees were protected from the rotating parts of the extruder of Line 1. On April 29, 2022, while the machine was in operation an employee's arm became caught in the rotating auger, resulting in a severe injury to the employee and the subsequent hospitalization of the employee.  b.  On or before May 4, 2022, the employer failed to ensure employees were protected from the rotating parts of the conveyor of Line 1. The guard on the conveyor had been previously removed and not reinstalled and was later found to be inadequate, thereby exposing employees to a caught-in hazard from the incoming nip point of the conveyor.

29 CFR  1910.219(f)(3):Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets.  a. On or before May 4, 2022, the employer failed to ensure employees were protected from the exposed chain and sprockets of the conveyor system of line 1. The chain and sprocket drive system for the conveyor on Line 1 was unguarded, thereby exposing employees to a caught-in hazard.  b. On or before May 4, 2022, the employer failed to ensure employees were protected from the exposed chain and sprockets of the winding machine. The chain and sprocket for the drive system of the winder was not guarded, thereby exposing employees to a caught-in hazard.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. On or before April 30, 2022, the employer failed to report a hospitalization of an employee to OSHA where an employee's arm was caught in an auger, resulting in a severe injury to the employee and the subsequent hospitalization of the employee.
