29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: (a) On or about May 5, 2022, at the job site located at W274N1490 Riverland Drive, Pewaukee WI, an employee working on a pump jack scaffolding was not utilizing fall protection which exposed him to a 18 8/10 fall hazard.

29 CFR 1926.454(a)(1): The employer failed to have each employee who performs work while on a scaffold trained regarding the nature of any electrical hazards, fall hazards and falling object hazards in the work area. (a) On or about May 5, 2022, at the job site located at W274N1490 Riverland Drive, Pewaukee WI, an employee working on a pump jack scaffolding was not utilizing fall protection which exposed him to a 18 8/10 fall hazard. The employee was not trained in fall protection when working from heights on scaffolding when greater than 10 feet.
