29 CFR 1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: a.) 200 Goodman Drive, Lewisberry, PA 17339 - on or about February 16, 2022, column F for case no. 2 on the 2022 OSHA 300 log did not provide a description of the injury. b.) 200 Goodman Drive, Lewisberry, PA 17339 - on or about February 16, 2022, column F for case no. 2 on the 2022 OSHA 300 log did not describe the part of the body affected. c.) 200 Goodman Drive, Lewisberry, PA 17339 - on or about March 20, 2022, column F for case no. 7 on the 2022 OSHA 300 log did not provide a description of the injury. d.) 200 Goodman Drive, Lewisberry, PA 17339 - on or about March 20, 2022, column F for case no. 7 on the 2022 OSHA 300 log did not describe the part of the body affected. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
