29 CFR 1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. Employees were exposed to fall hazard while decking roof approximately 19 feet high from the ground and each employee was not protected from falling as required.

29 CFR 1926.503(a)(2)(iii):The employer did not assure that each employee has been trained, as necessary, by a competent person in the correct use and operation of personal fall arrest equipment. Employees engaged in roofing operations were exposed to fall hazards on steep roof without use of fall protection training.
