29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Building Exterior-front: Employees were exposed to eye/face injuries while applying stucco without the use of eye and face protection. Employees were not protected with any type of eye/face protection, on or about 5/5/22

29 CFR  1926.451(a)(1):Each scaffold and scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it.  a) Scaffold/front:  Scaffold components were not capable of supporting, without failure it's own weight and at lest 4 times the intended load. Scaffold planks were damage and show of saw cuts, on or about 5/5/22.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a): Exterior front: Employees working on the first level of the scaffold were exposed to fall hazards of approximately 7 feet onto the ground below due to scaffold deck not being fully planked, on or about 5/5/22.  b) Exterior front right side: Employees working on the second level of the scaffold were exposed to fall hazards of approximately 13 feet onto the ground below due to scaffold deck not being fully planked, on or about 5/5/22.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.    a) Building Exterior Back: Employees were using pipe scaffold frames and cross braces to access the scaffold, exposing employees to a fall hazard of approximately 7 to 13 feet to the ground surface, on or about 5/5/22
