29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.   a) CNC Area - On May 5, 2022, an extension cord was placed across the floor in an area where employees walk through to power the Kalamazoo Belt Grinder, category #486-021, spec. #34C051-5506G14, creating a tripping hazard.  b) Deburring Area - On May 5, 2022, an air-line rubber hose was placed across the floor in an area where employees walk through to supply a portable sanding wheel, creating a tripping hazard.

There was damage to the front panel guard with an opening large enough for employees to reach through.
