An employee filled batteries with Alchemix QUAL-EZE (DILUTE ACIDS. 51-78%) without an eyewash station available for immediate emergency use.

The Yale propane powered industrial truck (SN N544692) was being operated while the emergency brake cable was broken.

A device such as a panic bar that locks only from the outside is permitted on exit discharge doors.  a) On or about May 6, 2022, the employer did not ensure that an exit route door on the south side of the building was able to be opened without key, tools, or special knowledge, exposing employees to hazards associated with not being able to exit the building in the event of an emergency. The door was locked with a slide bolt lock.
