On or about May 5, 2022, at several locations of the apartment complex, a) Building 1001 employees were working under or in the area below roofers and were not protected from falling debris with the use of head protection, exposing employees to struck-by hazards. b) Building 200 employees were working under or in the area below roofers and were not protected from falling debris with the use of head protection, exposing employees to struck-by hazards. c) Building private garages on the southeast side of the complex employees were working under or in the area below roofers and were not protected from falling debris with the use of head protection, exposing employees to struck-by hazards.
