29 CFR  1910.303(g)(1):Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) Warehouse- On May 3, 2022, the employer did not ensure that there was sufficient access or a safe working space around the electrical panels. The Square D electrical switchboard, main breaker cabinet, and electrical panels were blocked by pallets containing conveyor parts.   No abatement certification or documentation is required for this item.

29 CFR  1910.38(f)(1): The employer did not review the emergency action plan with each employee covered by the plan when  the employee was initially assigned to a job:  a) Warehouse- On May 3, 2022, the employer did not review the emergency action plan with employees, including temporary staffed employees from Banner Personnel Services, Inc. and Bureau Vertias, when the employees were initially assigned to the jobsite.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR  1910.110(f)(2)(i): Liquefied petroleum gas container(s) in storage were not located so as to minimize exposure to excessive temperature rise, physical damage, or tampering from unauthorized persons:   a) Bay Door, outside of Warehouse - On May 3, 2022, the employer did not provide an area for proper storage of the LP gas containers to minimize or prevent exposure to excessive temperature rise, physical damage, or tampering from unauthorized individuals. The LP gas containers were stored outside of the Bay Door of the Warehouse and were not secured to prevent from physical damage or tampering.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
