Main Street, LINN, MO 65051, on or about 5/9/2022, the employer failed to protect the workers by permitting them to operate a Terex XT5 Articulating Boom Platform from the bucket in elevated heights without any means of fall protection or restraining device with lanyard. The workers were exposed to fall hazards which could result in severe injuries or death.
