29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a.) On or about May 9, 2022, in the company's yard, employees performing, set-up, clean up and preventative maintenance on the KBR Shearline, Bender and Barwiser fabricating machines, during which a periodic/annual inspection of the energy control procedures was not conducted at least annually to ensure the startup or release of stored energy could not occur while servicing or maintenance on machinery would occur, exposing the authorized/affected employee and other employees in the machine area/danger zone to nip, pinch, cut, burn, amputate, crush and struck-by hazards.
