29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  On or about May 9, 2022,  on the north side employees were working under or in the area below roofers and were not protected from falling debris with the use of head protection, exposing employees to struck-by hazards.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.   On or about May 9, 2022, on the north side of the site, employees were performing roofing work at heights greater than six feet from the lower level without fall protection.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.   On or about May 9, 2022, on the: A) Northside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet      above the landing or secured.  B) Eastside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet      above the landing or secured.  C) Westside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet above the landing or secured.

29 CFR  1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall.   On or about May 9, 2022, employees on the north and west side of the site were using the ladder and carrying materials in their hands or shoulders while climbing the ladders. This could cause employees to lose their balance and fall from the ladders.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.   On or about May 9, 2022, on the north side of the site employee performing roofing work without fall protection. The employer was unable to provide documentation of fall protection training.
