On or about May 9, 2022, on the north side employees were working under or in the area below roofers and were not protected from falling debris with the use of head protection, exposing employees to struck-by hazards.

On or about May 9, 2022, on the north side of the site, employees were performing roofing work at heights greater than six feet from the lower level without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about May 9, 2022, on the: A) Northside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet above the landing or secured. B) Eastside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet above the landing or secured. C) Westside of the site employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet above the landing or secured.
