Employees had not been formally trained to operate the forklift and/or required to wear the seatbelt. (b) On or about May 6, 2022, and at times prior, employees operated a Hyster 80 forklift to unload, move, and/or load merchandise. Employees had not been formally trained to operate the forklift and/or required to wear the seatbelt.
