The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level. At an establishment located at 1102 Jefferson Street in Algoma, Wisconsin; the employer did not ensure that exit routes were free and unobstructed. Label making materials that come in during shipments were placed temporarily within the exit routes in the warehouse.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used as a substitute for the fixed wiring of a structure. On or about May 10, 2022, at an establishment located at 1102 Jefferson Street in Algoma, Wisconsin; the employer used a flexible cord as a substitute for the fixed wiring used to provide power to a switch and outlet hanging on the side of the 30-inch Rewinder in the Finishing Department. The flexible cord was hardwired into the switch and plugged into a Tripp-Lite SmartPro 120V Line-Interactive Uninterrupted Power Source (UPS). The outlet was providing power to a fan that blows trim waste from the Rewinder.

On or about May 10, 2022, at an establishment located at 1102 Jefferson Street in Algoma, Wisconsin, in the Finishing Department; the employer did not ensure that two flexible cords connected to a Tripp-Lite SmartPro 120V Line-Interactive Uninterrupted Power Source (UPS) were provided with strain relief to prevent pull from being directly transmitted to joints or terminal screws. The insulation on both flexible cords was frayed and/or disconnected from the housing of the plug.
